I do not quite understand your issue. 

1) In what are you opening the original PDF? (Adobe Reader directly or using
the Adobe Reader plug-in within a browser)?

2) What does the link point to? An HTML page? A PDF file? And how does it
point to it? What is the URL?

Certainly if you are using Adobe Reader as an application with a local file
and your external link is "http:" anything, you are instructing Adobe Reader
to launch the default viewer (a browser) to view an http: page, be it a PDF
or not. There is no way round that.

The same applies if you are viewing the PDF in the browser plug-in, it is an
http: request but it would be a replacement page to the one currently being
viewed (in the browser).

I want an <rx:bookmark external-destination="url()"/> to open in the same
PDF panel (window).  By default the link goes to the browser.  Is there
something I can set in the xsl:fo to have the PDF open in same panel?
