Yes, you can specify background-color on fo:external-graphic. This color
will be visible in transparent portions of the image, and around
the image if the padding is not zero.

We support transparency in GIFs. In PNG, we support single-color
transparency for grayscale and TrueColor images. For palette
(indexed color) PNG images, the first fully transparent color
is picked from the palette (only one). Alpha channel is not
supported.
NOTE: transparency works in PDF output only!
