[xep-support] width in fo:external-graphic

I noticed that when I use 'width' instead of 'content-width' in
<fo:external-graphic>, the image disappears! I guess 'content-width' is the
prefered way, but since both are legal, there ought to be some (other)
functionality for 'width' too. One way to implement it would be to let
'width' control the width, but not the height.
