To use headers reliably, you have to put keep-together on each fo:table-row.
I know it's a very serious limitation; unfortunately, this is hard to overcome
within current design.

This would also help. However, keep-together is inheritable, and specifying
it on rows is equivalent to speicifying it on cells.
