I think you also need to set the DocBook XSL stylesheet param 
'fop1.extensions' to zero.  Only one of the *.extensions params should 
be active.  See this doc for more information:

FYI, XEP uses its own extension element rx:outline instead of 
fo:bookmark-tree:
