Question #1 is are you using the latest version of the software. A bug fix
for a few tagged PDF items was released just in the last several days.

using XEP 4.21 build 20121114 with oracle jdk1.6.0_27 I am having trouble
using rx:pdf-structure-tag="Artifact" together with rx:alt-description on
different external-graphics within the same region.
eg.

<fo:flow flow-name="xsl-region-body">
   <fo:block>
     <fo:external-graphic src="url(image.png)" rx:alt-description="image1"/>
     <fo:external-graphic src="url(image.png)"
rx:pdf-structure-tag="Artifact"/>
     <fo:external-graphic src="url(image.png)" rx:alt-description="image3"/>
   </fo:block>
</fo:flow>

I would expect alt text for the first and third images, but the alt text
doesn't show for image 3. Am I doing this wrong or is there a problem here?
This behavior shows in Reader 9 and Acrobat Pro X.

In other more complex documents I am seeing in some cases (in the pdf) the
alt-text for one external-graphic showing on the next external-graphic from
the fo. I just presented the simple case here.
