[xep-support] keep-with-next; keep-with-previous doesnt work for block span="all"

Hello Friedrich,

Keep constraints do not work across boundaries of span="all" and 
rx:flow-sections (and never did). This is a limitation of XEP.

The only option is to manually insert breaks where appropriate, for 
particular document. There is no generic workaround.
