[xep-support] keep-with-next and span=all

Bonjour,
I try to attach a parapgraph title to the text who is on 2 columns.
I do something like :
<fo:block span=all> title </fo:block>
<fo:block>para 1</fo:block>
....
but somethimes there is a page break between the two blocks.
I read that this isn't implemented in older version of XEP is it always the case and is there a turnaround?
