[xep-support] Missed links in fo 1.1 index elements

Hi,

I have changed XEP index extension (<rx:page-index>, <rx:index-item>, 
etc) to the FO 1.1 tags (<fo:index-page-citation-list>, 
<fo:index-key-reference>, etc).

After generating of book, all page numbers are good, but links from them 
to referenced page is missed (when you using XEP elements you can click 
the page number, and you will be moved to this page).

Could you explain this? I am interesting of using FO 1.1 tags with 
links. What causing them to disappear?
