[xep-support] End of book index

[Dovid Zalkin]

 Hi. Using the XEP extensions for creating an end-of-book index, is there 
 any way to make the indexterm "hot" as well as the page number? (If 
 there are multiple pages, then I would want the indexterm itself to link 
 to the first page).
