XSL-FO 1.0 section 5.3.2 clearly indicates 
start-indent= has precedence in this case (even 
if inherited from ancestors).  In order to get 
padding= to have precedence over start-indent=, 
one would have to specify margin= and 
margin="0pt" is sufficient to engage padding in this fashion.
