[xep-support] Bug in rx:start-index-range and rx:end-index-range

Hi,

I realized that in some situations XEP is unable to find corresponding 
rx:start-index-range for rx:end-index-range. When processing attached FO 
file I'm getting following error:

{?rx:end-index-range: no id for ref-id='spanstart'}
error: formatting failed: null

But if you look inside FO document you will find rx:start-index-range 
with given id.

I'm using XEP 3.4 academic.
