I meant to write space-start (7.11.2), but it seem to be intended for
something else. Anyway, I see the point with using start-indent on the
fo:block now.

It's about layout on the first page. The main header has the same width as
the body, while the subheader has a 15mm indent. And so does the leader
between the main header and the
subheader. Something like this:

MAIN HEADER HERE
    -----------------
    Subheader here
