Er, no. You cannot incorporate any structure into our output PDF.
In my opinion, this is contrary to the very spirit of XSL FO; you
have your semantics in the original XML document, and bookmark/TOC
generation is automated by the stylesheet. Why would you want to use
Acrobat means for this? If you really need to share document structure,
share your XML sources :-).
