XSL FO does not provide a way to specify an image for the whole page.
When you specify a background image for the xsl-region-body it is placed
as background for the whole region, not for the each column. There is
no provision to specify a separate bg image for each column.

It is possible to implement almost anything as an extension and
if the time permits.
